Unfortunately I don't, and we keep over 40 lenders short sale packages on our members site. You may want to just use a generic financial worksheet and list of documents, we have one here you can use. Most all lenders don't mind if you use a generic one, they really just want ALL of the same docs. Feel free to edit it as you like.
